  2. You MUST fill them out completely including your license number BEFORE I can sign the slips. This will all be done virtually so they must be shared with me for signature.
  3. Once you have completely filled them out, if you have not already done so you must upload and add a signature field to the bottom right side of the CEU slip where it has a blank for the instructor signature.  Please spell my name correctly, Monika McGillicuddy and be sure to give me signing pliverages.  You may have to add me as a “person”  and if so my email is
  4. Once this is done, I will electronically sign the slip and share it back to you using the electronic platform you sent it through.
  5. Please do not send the CEU to me as jpg as we can not convert it. A pdf requires a few steps to convert so please avoid doing if possible.

Please log into the virtual Zoom classroom early, I would suggest at least 30 to 20  minutes early and test your system, the mic and camera so that you feel comfortable using Zoom.

Virtual CEU Requirements: Please read all the rules as without a camera on or mic on your computer you can not attend.

Zoom automatically records when you enter and when you leave the room thus only one person per-registration.  This record is printed at the end of the class and is the sign in sheet and proof of your attendance for the NH REC and shared with the REC upon their request.

Thus no one will get credit if they arrive more than 10 minutes late or leave early.

If you get booted out and re enter, Zoom will record that and your Attendance minutes will reflect how long you were gone for.

Please Do not plan on attending while driving your car, talking on the phone or while surfing the web and otherwise distracted.

* Please be sure to have your CAMERA On and Mute Yourself unless you have a question

Please download the Zoom app if you plan on attending via your phone and have your camera on so you can be seen

Please be sure to shut down all electronic media except what you are using to participate

in class.

This is a CEU class that is Live Instructor Led being transmitted Virtually and all the same rules apply as an in-person class

Important – After the class  ends the CEU slip will be e-mailed to all verified attendees. This will be filled out with all the class details but will need your information added and then my signature.

Please upload your completed CEU slip to Dotloop/Docusign.  Please add a signature box for the instructor and assign to Monika and then share it with Monika for signature.

The CEU slips will be signed upon verification of full attendance via the Zoom Class Record and sent back to you via the electronic platform you used.
